Greatix

This form should be used by trust staff, students and trainees to submit a nomination for an individual or team.

This should be in recognition of learning from excellence, for example where patient care or a process has been improved and where learning from an event has been put into practice. Submitting a Greatix enables us to capture positive events and improve safety by sharing and spreading the excellent work that happens at the trust.

Nominations as a “well done” or “thank you” should be submitted using the Moments of Magic form instead.

What happens next?

When you submit a Greatix form, the submissions will be reviewed by the Continuous Quality Improvement Team and the individual/team will receive a certificate of thanks from members of the Executive Team. The examples may also be shared on Pattie, in our trust Lessons Shared bulletin and through other means to spread the word!
