This form is for patients and their families and carers who are interested in being involved in improvement projects at Hull University Teaching Hospitals (Hull Royal Infirmary and Castle Hill Hospital). Ideally volunteers will be speaking from their own experience and be able to provide recommendations, ideas and feedback to support improvement across the trust. This is completely voluntary, and you are able to opt-out at any time by contacting us at hyp-tr.qualityimprovement@nhs.net at a later date. It is also unspecified how far you would be involved in the project – this could involve giving ideas for improvement, offering feedback on proposed changes or sharing your experiences as a patient, service user, family member or a carer. This is on a voluntary basis, and you are not obligated to be involved in any projects.
